When there is low secretion of T3 and T4 by the thyroid the TSH secretion by the hypophysis is very stimulated and the TSH blood level increases. The increase in the availability of the TSH promotes the enlargement of thyroid gland.The thyroid enlargement includes a reaction of tissue which tries to compensate the functional deficiency by making the gland increase in its size.
