Project Overview
You will be designing a very general database for a small provider's office. The office consists of one MD and two Physician Assistants.
The physician owner does NOT want a full EMR. He simply wants a database that he can use as a reference for the following questions/needs.
He wants to be able to look up his patients, their general demographic information such as DOB/gender/address/phone etc, their allergies, their insurance, and which provider they prefer to see.
He also wants to keep information on his providers such as their name, specialty, email and cell# information
He also wants to track his hours and which provider works which hours each day
He wants to be able to track the current pay rate for each provider along with the current amount of their PTO accrual.
Oops, he forgot to mention that he has two practice sites, so we need to track them and keep each address and phone number listed. This also means we need to track which office is the primary site for each provider.
Lastly, he needs to be able to track each provider's NPI#, and any payer-specific ID numbers for billing purposes.
