All patient-physician encounters must eventually be documented so that the medical billing team can translate the medical encounter data into an HCFA 1500 Insurance Claim Form for submission and payment.

To prepare for this Group Project, search the Internet for an HCFA 1500 form, such as the one found at the HCFA 1500 Claim Form. Also search for information on ICD-10 classification codes, as can be found at WHO ICD-10 Classification.

Do an Internet search for patient encounter forms, such as can be found at the AAFP - American Academy of Family Physicians.

This Group Project has two parts.

Part 1 is an individual effort.

Choose one of the patient encounter forms and describe in 3-4 paragraphs written in APA style:

• The different types of data requested on the form you selected.
• What subjective and objective data is requested and why.
• How the assessment component of the form is related to the ICD-10 classification.

Format your assignment according to the following formatting requirements:

1. The answer should be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides.
