Discussion: "The GrAde"
This phase, stage 4, reflects providers who use health IT to make the patient a true partner in his or her care. This elevation of patient to partner reflects not only the maturity of IT systems, but also of the patient's willingness to participate in shared decision making.
How does HIT facilitate shared decision-making and partnering? (at least 250-300 words)
