Imagine yourself as a hospital administrator ready to undertake a new quality improvement program for your hospital. In previous assignments you detailed aspects of your quality improvement plan for the facility and created your quality improvement approach for the facility. In this assignment you will demonstrate an understanding of the five domains of the performance measurement network in apply these components to improving healthcare quality services at your hospital.

Instructions

Create a PowerPoint presentation at least 15-20 slides in length that discusses your how your plan covers each of the five domains of quality (Access, Structure, Process, Outcome, and Patient Experience). Quality improvement in each domain should be explained and applied to the healthcare facility. Include detailed speaker's notes that describe your program. Be sure to include references and citations from at least five sources. Your PowerPoint presentation needs to be visually appealing as your goal is to thoroughly explain how each domain will be improved to your employees. You can illustrate this process by using one disease or condition of your choice presented to your facility by a hypothetical patient and how your new system would approach care of the patient in each of the five domains. Finally, explain how this new quality improvement program will help improve the healthcare employee experience.

This project will be presented in PowerPoint style, as you will be submitting your proposal to this group of hospital employees.

For this assignment, you will be drawing on the information that you put together in the unit 7 assignment. In unit 9 you will be looking at the program and quality improvement changes that you recommended to enhance the patient's experience.

The purpose of the assignment is to outline and identify how a quality improvement change can influence a patient's experience from start to finish

Based on the changes you suggested in unit 7 you will:

• Create a PowerPoint presentation at least 15-20 slides in length that discusses your how the plan covers each of the five domains (Access, Structure, Process, Outcome, and Patient Experience).

• Thoroughly explain how performance will be enhanced in each domain in the healthcare facility.

• Illustrate this process by using one disease or condition of your choice presented to your facility by a hypothetical patient and how your new system would approach care of the patient in each of the five domains.

• Explain how this new quality improvement program will help improve the healthcare employee experience.

Below is how you will address each question.

Questions 1 and 2

You are asked to discuss how your suggested changes will cover each of the five domains to include access, structure, process, outcome, and patient experience and then discuss how it can enhance performance.

I would recommend covering both of these parts together as you can list the domain, explain how it is covered, and then how it can enhance performance.

For example, if you suggested in unit 7 that the organization should implement the EHR you would explain how that might increase access to care, how it may change the structure of the organization, what processes will change, how it can improve outcomes, and ultimately what it can do to enhance the patient's experience.

It may be that nothing changes in a particular dimension - this is okay. You would simply explain why that no changes would be made.

Question 3

You will need to create a scenario, using a condition of your choice, and describe how the patient would move through the five domains. Keep in mind that nothing may have changed - that is okay as long as you explain why.

The condition you choose for the story does not really matter and should only serve as a way to guide the story

Below is an example for EHR implementation - do not copy this!

Access - a cancer patient enters the facility and is admitted to the hospital for a scheduled procedure - the patient was able to be checked in faster as a result of the EHR

Structure - the patient will not notice any difference in the organizational structure

Process - the process will not change but the patient will be able to move through the process quicker due to the electronic record being more efficient

Outcome - the patient's treatment was successful but the outcome was not directly changed as a result of EHR implementation

Patient experience - the patient is happier due to the shorter visit
