(a) An accident has occurred in hotel in which an employee is seriously injured. Illustrate out the stages or phases and the technique which should be adopted in the investigation of accident.

(b) According to Heinrich’s accident causation theory: ‘A preventable accident is one of five (5) factors in a sequence which results in injury’. Briefly illustrate out these five (5) factors.

Question 4:

Occupational Safety and Health (OSH), including compliance with Occupational Safety & Health Act 2005, are the responsibility and duty of the employer. The employer must show strong leadership and commitment to OSH activities in the organisation, and make appropriate arrangements for the establishment of an OSH management system. Policy, planning, organising and implementation, evaluation and action for enhancement play a key part in a proper health and safety management system. Explain how each of these steps must be performed.
