You will develop a proposal for review by the hospital leadership team. This proposal should contain the following (at minimum):
A. Executive summary detailing the need for collaborative care teams (CCT) and why they will benefit the hospital.
B. At least one paragraph to summarize each of the following components, citing relevant research as applicable. Assume fictional information to formulate your rationale:
a. Benefits of CCT to patients
b. Benefits of CCT to clinical staff
c. Structure of CCT (what providers are involved, and to what degree)
d. Oversight and assessment (who is responsible for tracking the success or failure)
e. Operational/Financial risks or areas of concern
f. Timeline for implementation
g. Conclusion: summarize key points to "sell" this proposal. Ideally, this will become a standardized model of care in your hospital, if a pilot is successful.
Format your homework according to the following formatting requirements:
o The answer should be typed, using Times New Roman font (size 12), double spaced, with one-inch margins on all sides.
o The response also includes a cover page containing the title of the homework, the student's name, the course title, and the date. The cover page is not included in the required page length.
o Also include a reference page. The Citations and references must follow APA format. The reference page is not included in the required page length.
